They have a form where I must write the card number and other credit card information.
they also ask for the credit cardholder’s signiture. but he’s not here and not accessible to me. What should I do?
The cardholder is a member of my family (a cousin) and he lives in the US. I live in Iran and can’t access him for a signiture.
can I sign on his behalf, or should I leave the signiture field empty?
what do I do?

Like When I was a kid, sometimes my Dad gave me his credit card and I signed for him. Never a problem.

Generally, you can sign on his behalf. If it was a stolen credit card, that would be considered credit card fraud, but an issue of fraud can only come to light if the card owner did not approve of such a transaction.
Therefore, if you have his consent, there should be no problem at all. Just make sure its OK with him first.
However, if it does not require a signature, simply do not sign it.

It is frowned upon to sign for him, but the signature is mainly for the card holder. If ever he got into a financial bind where he felt that somebody had used his card the bank can go back and look at signatures, and he can make sure that is his signature. I have used my parent’s credit card before and I just signed my first initial and last name… because our last names were the same. I usually tell them it’s my mom’s. So sign your initials rather than his name.

Signing for the person is illegal. Just get the “person” to write you a money order or something.
One more thing; it might look suspicious if your cousin signed in Iran while he was in the USA.
I hope you are not trying to pull a theft.
